FOREIGN MINOR CHILDREN OR FOREIGN CHILDREN WITH SPECIAL NEEDS REGARDLESS OF AGE, OF FILIPINO NATIONAL 

REQUIREMENTS FOR NON-IMMIGRANT VISA

  1. Foreign Minor Children or Foreign Children with Special Needs traveling alone or with Guardian

      1. Requirements must be submitted by mail or courier service. Do not use FedEx
      2. A duly-accomplished visa application form (Form No. 2). This form may be accomplished by a parent on behalf of the applicant. For People’s Republic of China (P.R.O.C) passport holders, use this form)
      3. Original and photocopy of applicant’s Birth Certificate issued by the County or State and the valid Philippine Passport or PSA/NSO Birth Certificate or Dual Citizenship documents of the Filipino Parent. (you will need to present this again in the Philippines)
      4. Original and Photocopy of both the Child’s Passport and Guardian’s Passport.
        • Both Child and Guardian’s passports must have a validity that must extend at least six (6) months beyond the length of proposed stay in the Philippines.
      5. One 2”x2” colored photograph with white background of the applicant taken within the last six (6) months.
      6. For foreign children with special needs, proof that Foreign Child is a child with covered by Section 4 (a) to (d) of RA 7277
      7. Notarized Affidavit of Request, Consent and Guaranty executed by either or both parents. This Affidavit must be presented before immigration officials at the port of entry in the Philippines. The Philippine Bureau of Immigration will issue a Waiver of Exclusion Ground (W.E.G.) upon the minor’s arrival in the Philippines
      8. Proof of Parent/s and Guardian’s financial capacity (e.g.latest print-out of credit card account(s), latest print-out of bank statements, ownership of real estate properties, and other documents showing the capacity to financially support said foreign child during his/her stay in the Philippines. You will need more than one type of proof of financial capacity
      9. Cashier’s check or US Postal Money Order pay to the order of the Philippine Consulate General Los Angeles in the amount of US$30.00 for the Visa Fee
      10. A photocopy of the applicant’s round trip flight itinerary (unpaid reservation) to his/her country of origin or his/her next country of destination.
      11. Letter request by the Filipino parent addressed to the Consulate with copy of valid identification card issued by the Philippine government
      12. Return envelope that is self-addressed with prepaid postage.

        • Marriage Certificate issued by the County or State and the Philippine Passport or PSA/NSO Birth Certificate or Dual Citizenship documents of the Filipino spouse.

REQUIREMENTS FOR IMMIGRANT VISA

      2. Two (2) copies of duly accomplished application form (F.A. Form No. 3).
      3. Original passport and two (2) photocopies of the applicant’s passport. validity must extend at least one (1) year beyond the length of proposed stay in the Philippines.
      4. Four (4) 2″ x 2″ size photograph taken within the last six (6) months. The applicant’s signature must be affixed on the front side of the picture.
      5. Medical Certificate (F.A. Form No.11) accomplished by a local physician. All supporting documents must be submitted in triplicate (three copies). A Chest x-ray plate in CD must accompany the certificate, together with the results for the following tests: 1) stool (ova/parasite and occult blood); 2) urinalysis; and 3) complete blood test (blood serology and chemistry).
      6. For foreign children with special needs, proof that Foreign Child is a child with covered by Section 4 (a) to (d) of RA 7277
      7. Apostilled Local State Police clearance obtained within the last (6) months. The original and one (1) photocopy must be submitted.
      8. Notarized Affidavit of Request, Consent and Guaranty executed by the Filipino parent. This Affidavit must be presented before immigration officials at the port of entry in the Philippines. The Philippine Bureau of Immigration will issue a Waiver of Exclusion Ground (W.E.G.) upon the minor’s arrival in the Philippines
      9. Proof of financial capacity (e.g. latest print out showing statement of credit card account(s), latest print-out of bank account statements, ownership of real estate properties, and other documents showing the applicant’s capacity to financially support himself/herself during his/her stay in the Philippines You will need more than one type of proof of financial capacity)
        • Sworn Statement of Financial Support executed by parents if proof of financial capacity cannot be complied with by applicant
      10. Original and photocopy of the Child’s apostilled Birth Certificate
      11. Original and photocopy of the proof of parent’s Filipino citizenship (e.g. Philippine passport or Philippine birth certificate)
      12. Letter request by the Filipino parent addressed to the Consulate with copy of valid identification card issued by the Philippine government
      13. Return envelope that is self-addressed with prepaid postage.
      14. Additional requirements depending on who will travel with the minor:
        • Traveling with Guardian:
          1. Original and photocopy of applicant’s Birth Certificate issued by the County or State and the Philippine Passport or PSA/NSO Birth Certificate of the Filipino Parent. (you will need to present this again in the Philippines):
          2. Original and Photocopy of both the Child’s and Passport and Guardian’s Passport.
            • Both Child and Guardian’s passports must have a validity that must extend at least six (6) months beyond the length of proposed stay in the Philippines.
        • Traveling with Foreign Parent married to Filipino parent
          1. Original and photocopy of Proof of Marriage such as (you will need to present this again in the Philippines):
