Application for a Travel Document

A travel document is allowed only for applicants who cannot be issued a Philippine passport and have an emergency travel to the Philippines such as due to medical or legal reasons or death in the family.

It is valid for 30 days from issuance and can only be used one-way to the Philippines. In order for you to leave the Philippines, you will need to apply for a new passport.

 

REQUIREMENTS

  1. Travel Document Application Form [ click here ] .
  2. Print out/Copy of Valid Airline Ticket with departure date of less than one month from date of applying for a travel document;
  3. 4 pieces of 2×2 photos with white background taken at most six (6) months from date of applying for a travel document ;
  4. Original or photocopy of Proof of Urgency (e.g, Medical Certificate, Death Certificate, Subpoena Ad Testificandum)
  5. Old Philippine passport;
  6. Cashier’s check or Money Order pay to the order of the Philippine Consulate General Los Angeles in the amount of  US $ 30.00
  7. Return mailing envelope that is self-addressed with sufficient prepaid postage for tracking number (e.g.  USPS Priority / Priority Express Mail Envelope. Do not use FedEx).

If the airline ticket purchased for travel to the Philippines has a departure date of less than 48 hours from purchase date, email passport@philippineconsulatela.org

The Consular Officer reserves the right to require additional documents pursuant to the Philippine Passport Law and the Foreign Service Act .

REMINDER: 

In applying for a new passport in the Philippines, surrender the Travel Document issued by the Philippine Embassy or Consulate together the affidavit of lost passport (as the case maybe) executed at that Post.
