Procedures and Requirements:
1. Four (4) original duly accomplished Report of Death form (either type written or legibly handwritten) should be executed by next of kin or the attending physician or nurse or their duly designated representative. When the death is reported before the Consulate, the Report of Death will be sworn to before a consular officer. If the Report is sent through mail, the Report of Death form must be notarized with the notary public signing AND acknowledging that the one reporting the death personally appeared before the notary public. 2. The original copy of the Certificate of Death issued by the local health authorities or by the local County Registrar must be presented. The Certificate of Death should also be reproduced in quadruplicate (four copies).
3. Valid Philippine passport of the deceased must be submitted, also reproduced in quadruplicate (four copies), in order to determine Filipino citizenship. In the absence of the valid passport, any other form of identification showing Filipino citizenship such as (a) Dual Citizenship (RA 9225) Documents, (b) NSO/PSA Birth Certificate [you can apply for the birth certificate here ] with NOTARIZED affidavit of next of kin certifying that the deceased has not acquired foreign citizenship or (c) expired Philippine passport with NOTARIZED affidavit of next of kin certifying that the deceased has not acquired foreign citizenship.
4. The fee for this service is $25.00 in cash, postal money order or cashier’s check payable to the Philippine Consulate General. Personal checks, peronal money orders, ATM and credit cards are not accepted as mode of payments. Do not send payment in cash when reporting by mail. An additional mailing fee of $7.00 is required if the Report will be returned by mail, unless you provide a USPS Priority Mail Flat Rate Envelope (size: legal or 12-1/2″ x 9-1/2″) that is self-addressed and postage prepaid.
Please take note of the mailing or courier tracking number since the Philippine Consulate General cannot assume responsibility for documents which have been mailed through the U.S. Postal Service or courier service.

b. APPLICATION SUBMITTED THROUGH MAIL OR COURIER – Report of Death is mailed within five (5) working days after receipt by the Consulate of the duly accomplished application form and all requirements.
